What are Case Management and Coordination Services?

What do we do?
Case Management Services link consumers to service, monitor the service
provision, assess for changing needs, and advocate for consumer preferences
and choices.
How do I make a referral to one of these programs?
Referrals for case management services can be made by contacting the Access
to Care team at (434) 847-8035 for an appointment. Once a case manager has been
assigned, he or she will assist the consumer in accessing other services
provided by CVCS.
