Case Management Services link consumers to service, monitor the service provision, assess for changing needs, and advocate for consumer preferences and choices. How do I make a referral to one of these programs? Referrals for case management services can be made by contacting the Access to Care team at (434) 847-8035 for an appointment. Once a case manager has been assigned, he or she will assist the consumer in accessing other services provided by CVCS.
