Case Management Services

MR Case Management services are provided to continue support and linking of services for those children 0-22 with mental retardation and for those children 0-5 who have mental health issues. They work extensively with the families of these individuals to maintain stability in the home.

The team provided services to 250 consumers. They were able to secure fourteen new waiver slots this fiscal year. Three of the fourteen were reallocations due to death or move.

MH Case Management under Specialized Outpatient Services was provided to 950 children this fiscal year. This number increased from 770 children served in the 2002-2003 fiscal year. With the increase in staff we were able to eliminate any waiting list for MH case management and increase our coverage of the counties of Appomattox, Amherst, Bedford, and Campbell. Each county now has an assigned case manager in addition to a therapist(s) for that locality. The case management staff members have been a critical part of our success of reducing the waiting list and providing timely services. They have also been instrumental in filling the gaps for consumers who were waiting for a therapist. The staff also assisted in providing group treatment to our MH Anger Management groups for both boys and girls. In addition to the clinical benefits of providing timely services within the various localities it has made a positive fiscal impact on our budget.

MH and MR Case Management have been provided in 29 schools this year. They have maintained an ongoing community presence by staff attending PLAN meetings and the Day Care Task Force. The teams continue to have difficulty in obtaining services for dually diagnosed children. It is particularly difficult to find placement for this population and is very costly to the community. The staff also participated in the Transitions meeting to facilitate the transition for consumers from Child and Family to Adult and Family Services.
