What do we do?
Case Management Services link consumers to service, monitor the service provision, assess for changing needs, and advocate for consumer preferences and choices.
In-Home Support Services may include training in activities of daily living and self-help skills, money management, or personal assistance, and are usually provided in the consumer's home or during community visits.
Who do we work with?
Case Management Services are provided for all clients who are open to the agency, and who have a mental health or mental retardation diagnosis.
In-Home Support Services are provided for clients who are eligible for services as a result of a diagnosis of mental retardation, and who have needs that can be served by the program.
Respite Services are provided for clients who are eligible for services as a result of a diagnosis of mental retardation.
How do I make a referral to one of these programs?
Referrals for case management services can be made by contacting the Access to Care team at (434) 847-8000 for an appointment. Once a case manager has been assigned, he or she will assist the consumer in accessing other services provided by CVCS.
